A doctor comes to the home, confirms the death and issues the medical death certificate. Every day from 8:00 to 24:00.

When someone dies at home, the family needs a medical death certificate to continue with the paperwork. At SmartDoctor® a doctor comes to the home, confirms the death and issues the certificate during the same visit, with respect and without rushing.

When we can issue it

The doctor can issue the certificate when the person had a known diagnosis or a treating doctor that explains the cause of death.

If the death was sudden with no known illness, or caused by an accident or violence, the doctor cannot issue the certificate: in those cases call the police (Carabineros, 133). If you are not sure the person has died, call 131 (SAMU) immediately.

Documents to have ready

  • The deceased person's ID card or passport.
  • A medical document describing their illness or current condition, for example a hospital discharge summary (epicrisis), a report from their treating doctor or a palliative-care order.
  • Recent prescriptions or test results, if you have them.

Fee and hours

  • Fee: CLP $120,000. You receive a receipt.
  • Hours: every day from 8:00 to 24:00.
  • Districts: Providencia, Ñuñoa, La Reina, Las Condes, Vitacura, Lo Barnechea, Peñalolén, La Florida, Santiago, Macul and Huechuraba. If your district is not listed, message us and we will confirm whether we can reach you.

How to request it

  1. Message us on WhatsApp (+56 9 5675 8880) with the district, the address and a short description: the person's diagnosis and whether you have a discharge summary or medical report.
  2. We confirm the doctor and the arrival time.
  3. The doctor confirms the death, reviews the records and issues the death certificate.

With the certificate, the family or the funeral home then registers the death at the Civil Registry (Registro Civil).
